I heard that Mafia and Cosa Nostra aren't the same, it said that the Cosa Nostra is Napolitan, and isn't the same as the Mafia. Is this true?

It's generally the Camorra that are from Neopolitan descent. Cosa Nostra definately is the Sicilian/Italian Mafia in America which basically translates as "Our Thing". John Gotti was recorded on an FBI wiretap as saying he'll be "Cosa Nostra till I die".

I don't know if this will apply to the Sicilians in Sicily though or even the Italians on the main land. Usually the Sicilians refer to each other as "Men of Respect" or "Friends". I don't know the translation for these though.

"Mafia" and "Cosa Nostra" are used interchangeably in the US. "Mafia" was known for decades. Joe Valachi first used "Cosa Nostra" during his televised Senate testimony in 1962. He said it was the term ("our thing") that Mob guys used among themselves in referring to the Mafia.
