There are rules in the maffia about not killing children and women. But every maffia-rule has been/is broken, including that one. In Sicily, they didn't do it until the Corleonesi took over, they were really brutal, and also killed children.

The rule of not being allowed to murder any family members of your enemy was introduced by a certain mobster (whose name I can't think of now) when one of his Soldiers was murdered in front of his family.

Innocent relatives of Tomasso Buscetta were killed by the Corleonesi. They were not in the mafia.

Also the child (16 years) of Inzerillo was killed by the Corleonesi because he told he would take revenge for his fathers murder.

Paolo from The Godfather Part II could possibly have been based on this child who was murdered by the Corleonesi.
