1) Like all gangsters, Roth was greedy. He wanted to be sure he got the 2 million prior to killing Michael. 2) The ascot (scarf) on Michael's neck was probably just a just a fashion statement (a poor one  ). 3) The pills were more than likely a foreshadowing of Michael's diabetes.

...except that GFIII was not envisioned at that time. I will guess that the pill was Milltown, the ubiquitous tranquilizer of the late Fifties and beyond. (I'll further guess that it was the drug Geary swallowed in Michael's office during Anthony's party. It was also the drug referred to in the Rolling Stones' '65 hit, "Mother's Little Helper.") Tranquilizers were new in the Fifties. Taking Milltown was a sign that you were a "complex" person and experienced "stress" in your busy/busy/important life that deserved to be alleviated pharmecutically.

I know it doesn't count because it wasn't filmed, but in one of the early GF2 scripts, wasn't there a scene at the end of an 18 year old Anthony visiting with Michael and a mention of Connie treating Michael for his diabetes? That's why I always assumed the pills may have been for his blood sugar, because more often than not diabetics are on the pills long before the insulin. Just a thought.
Back to that ridiculous ascot: Is it possible that Michael wore it to look more regal, like the Lord of the Manor, to further distance himself from his father's "peasant" beginnings?
If he did, it would be a kick in the ass, considering that he died in the same peasant clothing as his father. And his father didn't have dogs peeing on him!

I thought there were foreshadowings of the diabetes as early as GFI because Michael is always drinking water. Throughout all three films he drinks lots of water and rarely drinks alcohol.
That ascot was totally out of place. Did Michael think he was British nobility? I liked him better with his shiny silk suits and his oily hair.

Dunno about an earlier script mentioning Michael's diabetes, but I've heard it referred to here. I like my Milltown guess because it's so...Fifties.  It really is a "fifties" drug. Do they still make it or has Valium and Xanax totally relaced it? One problem, however. Would Michael's need for Milltown show that he was tense, and therefor somehow weak? See, e.g. De Niro's fear of psychotherapy in "Analyze This."
