For Matteo Messina Denaro to be able to stay on the run for going on 3 decades now, Cosa Nostra still has to be extremely, extremely powerful. There's no way around it. He's one of the most wanted men in the entire world. There's proof that he's even been visiting many other countries too which is nothing short of extraordinary in this day and age, and it's even been rumored that he has visited the United States. I don't know if that's true, but whenever he's finally caught, I think the revelations that come from it will blow your socks off. I think the fact that Messina Denaro is on the run for that long isn't so much because of his personal power, but because the "higher-ups" protecting the mafia (politicians, corrupt law enforcement, freemasons etc) need him as a sort of "boogeyman" to divert attention from themselves. If he was arrested, many people would say "ok, the last defendant in the bombings/massacres of the 90s has been caught, now let's focus on whoever protected him and is still free", and that's what many powerful people don't want. About the potential revelations in case of his arrest, I can't know for sure of course, but I think he only will be arrested when his replacement is ready, and whatever evidence and compromising documents he may have, they surely will already be safely hidden long before the arrest will be "allowed" to happen. When Riina was arrested, the police took away the surveillance from his home and allowed the mafiosi to remove all compromising evidence from it before the house was searched. When Provenzano was arrested, no big revelations came either...

He is a billionaire for sure, he is most powerful organized crime figure in Europe , maybe next to some Russian mafia bosses who are close to Putin. But as Dwalin said , after arrest of Riina , someone took away surveillance from his home , when they came back , house was empty.

Italian police seize mafia goods worth 800 million euros in Calabria Italian police announced on Wednesday that they had seized property worth more than 800 million euros. ‘NDRANGHETA: AIDING AND ABETTING BOSSES WHILE ON THE RUN, 8 ARRESTS
(ANSA) – REGGIO CALABRIA, MARCH 10 – Eight people were arrested by the Reggio Calabria mobile squad accused of having favored the inaction of the boss of the ‘Ndrangheta Giuseppe Pelle, known as “Gambazza”, considered one of the alleged leaders of the homonymous cosca of San Luca. Six of the arrested are relatives of Pelle, accused of having helped him escape the execution of a prison order under which he had to serve a residual sentence of 2 years and 5 months in relation to a definitive sentence for mafia association. The other two involved in the operation are not relatives of the boss, but would have been his supporters anyway.
The operation conducted by the Police was called “Defender”, from the name of the off-road vehicle on board which Giuseppe Pelle, while on the run, was accompanied to the Condofuri hideout where he had found refuge and where he was arrested on 6 April 2018 by same mobile team of Reggio Calabria and the Sco. The precautionary custody orders carried out as part of the operation were issued by the district magistrate of Reggio Calabria at the request of the Public Prosecutor, Giovanni Bombardieri, and the Deputy Prosecutor, Giuseppe Lombardo. The detention in prison was arranged for his wife, Marianna Barbaro, aged 55, and two sons of Pelle, Antonio, aged 35, and Francesco, aged 31; for the son-in-law, Giuseppe Barbaro, aged 36, and for his nephew Antonio Pelle, also 36 years old.
