The Grimaldi family but they are said to have been shelved, the Sabella family, The Galante family, the Profaci family, the Colombo family, Aloi, Cammarata, Persico, Corrao, Gambino, Indelicato, Mancuso, Mannino, Mosca, Santoro, Amato, Arcuri, Riccobono. There are many and they dont belong to just one family. If you are asking the family remained and passed the rank onto another family member going back before or at the time of the naming of the five bosses and still on the street today, then the Corrao family in the Gambino crime family, the Corrado and the Tocco families of Detroit are the only other families I can think of today. There are examples of it happening in the past, but after the commission trial, and strawman cases it became rare for that to happened.

Salvatore Scafidi in Philadelphia is at least a 4th generation wiseguy, with Gaetano 'Big Tom' Scafidi being an early member from the 1920s the Scafidi family ended with Tommy 'Horsehead' Scafidi turning informer. I can only think of the Bonventre family.

GV on the above how many of them still have a relative active currently ? Just realised I’ve repeated a few of the ones you already mentioned
