When Michael arrives back home from Cuba, he's in a hotel room with Al, Rocco and Tom. (the scene where he asks Al for a wet towel and Tom tells him about Kays 'miscarriage'), what city are they in? Tom says that Roth is in a hospital in Miami which leads me to believe THEY are not. I always thought they WERE. Any thoughts?

The scene you're referring to is PROBABLY set in Las Vegas. The tumbleweed, dust and trees suggest desert, not Miami. It's also warm enough that the people in the scene don't need winter gear. In the NEXT scene, Michael returns home to Tahoe, which is well to the north, and experiences snow and cold, as shown. It can get chilly at night during the winter in Vegas, but snow would be very rare.
