Rough Part IV outline

- Picks up shortly after Kay's death (about 3 years since Mary's death)
- Vincent is still the Don.
- Vincent wants Michael as consigliere, in hopes to avoid all the tragedies that Michael experienced.
- Michael objects, moves to Sicily.
- Vincent moves ahead with business, which involves being the minority owner of a fictional New York baseball team.
- When the majority owner of the team wants to move to Las Vegas, Vincent objects. Vincent wants the team to stay in NYC.
- Vincent slowly comes to realize that the owner of the baseball team and a powerful casino owner in LV are conspiring against him.

- Subplot involves Anthony's fall from grace as an opera singer to a drug addict.
- Anthony is helped by a famed NYC caregiver who nurses him back to health and saves his opera career.
- The caregiver is a distant relative to Don Cicci, who is seeking revenge on the Corleone family for murdering his ancestors and ruining the family name. The caregiver has joined forces with the baseball owner and the LV casino owner
- When Anthony realizes the plot against his family, he joins Vincent against Michael's wishes.
- As the plot to kill Vincent evolves and eventually results in a failed assassination, Vincent and Anthony flee to Sicily and join Michael who reluctantly takes them in.
- There, Vincent, Anthony and Michael (still reluctant) plot out Vincent and Anthony's revenge.
- Vincent falls in love with a native Sicilian girl and marries her.

- Vincent and Anthony return to the states and seek revenge on baseball owner, LV casino owner and NYC caregiver.
- Michael surprises Vincent and Anthony at the Corleone Estate after the assassinations on the baseball owner, casino owner and caregiver.
- Michael pleads for Anthony to leave the family life, join him in Sicily and pick up his opera career.
- In the end, Anthony, who was never to be associated with the family, per Michael's wishes, has become a part of the family, thus setting up a potential power struggle between Vincent and Anthony in a Part V.
- The final scene shows Vincent and Anthony together in a den, conspiring against future enemies. They look up at Michael who stands outside the den. The door is closed in his face.
