In The Godfather Part II, Willi Cicci says "No, that was no heart attack" when Frederico and Frankie Pentangeli are discussing the death of Peter Clemenza. How did Peter Clemenza actually die, if the real cause of his death is known?

The Slippery Slope
We are never told the real cause of Clemenza's death, but presumably the Rosato Brothers had something to do with it, either by speeding up some "natural cause" or by some manner of direct murder.

No, as far as I know there is no hint in the Trilogy about the cause of Pete's death. Maybe there is an alternate script out there that describes it.

As I have maintained in other posts, I believe that some of the characters in GFI & GFII are at least partially based on real life Mafiosi. When it comes to Clemenza I believe that he is based on Joe Profaci who did die of a heart attack. Profaci was allied with Joe Bonanno in the 50s and 60s. Profaci was succeeded by Joe Magliocco who I believe is represented in the Trilogy by Pentangeli although Magliocco never turned on his fellow Mafiosi nor committed suicide.

Clemenza had a heart attack when they told him they weren't going to make him the highest paid in GFII

I don't think they really felt the need to explain it. The line is only there to prevent any awkwardness towards his absence, which is actually there just because he asked for an absurd contract...

"Somebody told me when the bomb hits, everybody in a two mile radius will be instantly sublimated, but if you lay face down on the ground for some time, avoiding the residual ripples of heat, you might survive, permanently fucked up and twisted like you're always underwater refracted. But if you do go gas, there's nothing you can do if the air that was once you is mingled and mashed with the kicked up molecules of the enemy's former body. Big-kid-tested, motherf--ker approved."

It surprises me that Clemenza's death wasn't ever brought out into the open, the same applies to Tom Hagen.

That's one place where the second two movies could have done a better job. The legends from Part I we all learned to love are suddenly gone and there are suddenly new wiseguys around. I think that's why Winegardner's book sucked, he brought in all these characters we didn't know (or give a shit about) and focused less on the characters we already know and have taken an interest in.

I dont think it was about the money,Castellano wanted his agent to write his own script for clemenza, and FFC could not make a successful movie if clemenza's lines werent the same as he wanted.

It wasn't "no heart attack" but maybe it was a stroke. He didn't look like the kind of guy who ever passed by a cannoli he didnt eat.
