TB I always thought she was trying to be as cruel and hurtful to him as possible in making that remark. He was outwardly still trying to be accepted in the legitimate world, and Kay,IMHO, with her WASP New England Background and her "respectable" second husband was making a crude slur at his being Sicilian and a "common mafia hood."
