Like m2w said,there is a boss of bosses according to police, so they are all under one boss/or multiple people who are on top. Ndrina boss answer to someone higher than he is.
1)ndrangheta isn't an exstension of Cosa Nostra; 2)the had no boss of the bosses but a chamber of control for avoid the feuds.
I said some believe they are extension of Sicilian Mafia, even Buscetta made that claims.
